家庭自負額
符合托育補助資格的家庭每週應向托育人員直接支付家庭自負額。每週家庭自負額的計算方式如下：家庭總年收入中*高於聯邦貧窮線*的部分乘以 1% 再除以 52 週。

1. 由此下載州申請書 PDF 檔（無法直接填寫，請列印出來）。
2. 致電 607-274-5612 請專員將紙本申請書郵寄給您。
3. 親自到伊薩卡市中心的 Human Services Building 接待窗口領取申請書（地址：320 W. Martin Luther King, Jr./State Street，公立停車場位於 Seneca Street，Albany 和 Plain 之間。
