The CYSHCN program serves families with Children from birth to age 21 who have or are suspected of having a serious or chronic physical, developmental, behavioral, or emotional condition, and who require health care and related services beyond that which children generally need.
Chronic diseases remain a leading cause of illness and death in Tompkins County, with inequities in prevention, early detection, and management contributing to avoidable health disparities. While overall indicators in some areas appear favorable, gaps in access, structural barriers, and differences in care utilization continue to disproportionately affect communities harmed by racism and social injustice.
Our CHWs are dedicated to identifying needs and connecting people to services. They go directly to residents and community groups to provide on-going connection and support.
The Healthy Neighborhoods Program (HNP) provides free home safety visits to promote healthy homes. HNP is part of the Health Education Program at Whole Health, and is funded by a grant from the NYS Department of Health. HNP visits are conducted in-person or virtually, with option for drop off of home safety products based on client preference.
